The Care Plan is auto-enrolled and completed every 6 months, with the first being done within 30 days of enrollment. The customer must provide appropriate level of access to their Salesforce system. The Care Plan is billed upon completion and submission of Diagnostic Results. Customers will be required to provide a Credit Card on file that is automatically billed. Customers are required to submit an email more than 30 days prior to the next Care Plan to request cancellation.
